Students interested in joining a Discovery Camp need to complete this Application Form.
Application form includes:
- Health and medical information including pre-existing medical conditions, pre-existing injuries, medication, allergies and any other key info.
- Dietary requirements such as gluten free, dairy free, vegan, Halal etc.
- Emergency contact info
- Informed consent to participate in the programme
- Personal motivation
- Interests and goals
